Portable Medical Record (PMR)
We’re excited to offer a new service to
our patients. Now you can request a free Portable Medical Record (PMR) on a
mini-CD!
What is a Portable Medical Record or PMR?
If you are admitted to the hospital, the medical staff may need
your health information right away. You may not always have this information
readily available. By having your PMR CD with you, you can save valuable time.
When your CD is loaded into the hospital computer and you enter your password,
you and your doctor will be able to access your information in an instant. It’s
that easy.
What kind of information is on my PMR? Your
PMR has a summary of your key health care information
including:
• Current medications and health
problems • Test
results • Recent
hospital stays •
Recent EKG results •
Allergies • Your
advance directive (if you have one) • Contact information including names and phone numbers
of people you would like the health care workers to notify
Why do I need a PMR? If you are

How do I get my PMR? Getting a portable medical record is simple. Just follow these four
steps:
• Log on to smalv.com and download the PMR Participation Agreement
Form or pick one up at any SMA health care center. • Fill out the PMR Service
Participation Agreement Form. • Bring the signed form to any SMA health care
center, or mail it to: Southwest Medical
Associates P.O. Box

89114-5645 • Watch for the CD to arrive in the mail within
two to four weeks. Your password will be sent separately.

PMR-CD Authorization Form
How do I update my health information on my
PMR? When used with your password, your
CD will automatically show you a summary of the most recent information in your
SMA medical record.
What if I lose my
PMR? Call the SMA PMR
hotline at (702) 877-5020 as soon as possible. Your CD will be
deactivated to stop anyone from possibly gaining access to your health
information. You will quickly get a replacement CD by mail. Your password will
be mailed separately.
Important Note: This program is voluntary. An advance
directive is not required to receive health care at SMA or to receive any health
plan benefits.
