Making Your Appointment Better
Before Your Appointment Once you've made an
appointment with your provider, especially for a first-time visit, we recommend
you bring the following items with you to your appointment:
- Your health insurance card and
picture ID (driver's license, etc.).
- Your prescription bottles or a list
of your medications including herbs and vitamins. This list should
include the name of the medication and the dosage, as well as the provider who
prescribed it to you.
Print and fill out the Making
My Appointment Better Form, and a copy of your advance directive.
Checking In At check in, you’ll be asked to confirm
your address, phone number and insurance information. If this is your
first visit to SMA, you will need to complete a general medical history
form. Our providers know your time is important. Please check with
the front desk staff if you have not been given an update on the status of your
appointment after 15 minutes. Your provider may be behind schedule due to
unexpected patient needs.
During Your Appointment The nursing staff will
check your weight, temperature, pulse and blood pressure. You’ll then be
asked general health questions. This is also a good time to share your
health questions. You and your provider will discuss your key concerns, as
well as current prescriptions and any necessary follow-up or referrals.
During the exam, your provider will review and update your medical record
electronically. This allows for real-time record access and changes.
At the end of your visit, a member from the nursing staff will provide you with
your check-out paperwork and refer you to the front desk to complete your
appointment.
After
Your Appointment We want to meet your expectations and invite
you to complete a Patient Comment Card and tell us about your experience.
You can find a Patient Comment Card at most of our courtesy desks and patient
check-in areas.
