Before your appointment
- Write down the date, time and location of your appointment.
- For primary care appointments, print out the new patient packet. Fill out the information and bring it with you to save time.
- Download our easy-to-use appointment checklist. It will tell you what to bring and what to expect.
- For specialty care appointments, make sure your primary care provider’s office processes your referral before your visit.
- Plan to arrive 15 minutes ahead of your appointment time.
- Bring your prescription bottles or a list of your medications, including herbs and vitamins. This list should include the name of the medication, dosage and the provider who prescribed it to you.
- Bring any previous, relevant medical records you may have.
- Follow any pre-appointment instructions you may have been given (fasting, liquids, medicines, etc.).
- Before you leave, make sure you have your health insurance ID card and picture ID (for example, your driver’s license).
Check-in
- Check in at the front desk and confirm your address, phone number and insurance information. You’ll pay your copay before your appointment. You will need to provide a valid ID and health insurance card.
- Complete a general medical history form (first-time visits or yearly for some services).
- Feel free to check with the front desk staff if you haven’t been updated on your appointment after 15 minutes. Your provider may be behind schedule due to unexpected patient needs. We understand your time is valuable.
During your appointment
- First, the nursing staff will check your weight, temperature, pulse and blood pressure.
- You will then be asked general health questions. This is also a good time to share your health questions.
- You and your provider will discuss your key concerns. You’ll also talk about current prescriptions and any necessary follow-up or referrals.
- Your provider will review and update your medical record electronically during the exam. He or she will help you with next steps for any preventive exams or tests that are due.
- A member from the nursing staff will provide you with your check-out paperwork. You’ll go to the front desk to complete your appointment.
Some things to remember
- If you can’t make your appointment, please call the scheduling line to cancel it. This will help us use that time for another patient.
- Make sure to get any needed tests done in advance. This way your provider has them to review at your appointment.
- Be honest with your provider. Make sure to tell him or her any health concerns you might have.
